Cagrilintide (development code AM833) is a synthetic, lipidated, long-acting analogue of the pancreatic peptide hormone amylin. Novo Nordisk developed it to fix two weaknesses of native amylin: a strong tendency to form amyloid fibrils and very short persistence. It is a non-selective agonist at the amylin receptors (AMY1–3) and the calcitonin receptor (CTR). Structure
Native human amylin (also called islet amyloid polypeptide, IAPP) is a 37-amino-acid peptide with a disulfide bridge near its N-terminus and an amidated C-terminus. It readily aggregates into amyloid fibrils, which makes it hard to work with. Cagrilintide keeps the amylin architecture but changes it in two ways:
- Anti-fibrillation substitutions. Amino-acid changes along the chain reduce the peptide’s tendency to form fibrils and improve chemical stability in solution (Kruse et al., 2021).
- N-terminal lipidation. A C20 fatty diacid on a γ-glutamic acid linker binds serum albumin, which extends the peptide’s persistence in experimental systems.
Kruse et al. also solved a crystal structure of the cagrilintide peptide backbone fused to maltose-binding protein (PDB 7BG0).
Mechanism studied in research models
Amylin acts through receptor complexes. The calcitonin receptor (CTR), a class B GPCR, pairs with one of three receptor activity-modifying proteins (RAMP1, RAMP2 or RAMP3) to form the amylin receptors AMY1, AMY2 and AMY3. Cagrilintide activates these complexes and CTR itself, so it is classed as a non-selective amylin/calcitonin receptor agonist. Like other class B GPCRs, these receptors signal mainly through Gs and raise intracellular cAMP. Researchers use cagrilintide to study amylin-receptor pharmacology with a ligand that is more stable than native amylin.
